Prime Care Nursing & Support Services, LLC provides skilled pediatric nursing services through the Georgia Pediatric Program (GAPP) for children who require ongoing medical care in the home. Our experienced pediatric nurses understand the unique needs of medically fragile children and their families.

What Is the Georgia
Pediatric Program?
GAPP (Georgia Pediatric Program) is a Georgia Medicaid program designed to provide skilled nursing services for medically fragile children under the age of 21 who require ongoing medical care that cannot be provided by a family member alone.
Medicaid-Funded Program
Available for eligible Georgia Medicaid beneficiaries with qualifying medical needs
Physician-Ordered Services
All services are based on medical necessity and require orders from your child's physician
Flexible Care Settings
Care can be delivered in the home, school, or approved community settings
Family-Centered Approach
We work as partners with families to support your child's unique care needs
Our Pediatric GAPP Services
Comprehensive skilled nursing and personal care services tailored to your child's medical needs and care plan.
Skilled nursing visits and shift nursing
Personal care services (bathing, dressing, feeding assistance)
Medication administration
G-tube and enteral feed management
Tracheostomy and ventilator care
Seizure monitoring and safety protocols
Respiratory support and oxygen monitoring
Care coordination and documentation support
Parent/caregiver education and training
Parent Caregiver Option
In some cases, parent caregivers may be able to provide personal care services under GAPP based on qualifications and insurance approval. This allows trained parents to be compensated for providing care to their own child.

Requires completion of required training and competency validation
Personal care services only (not skilled nursing procedures)

Transfers Welcome
If your child is currently receiving GAPP services through another agency and you're considering a change, we're here to help make that transition smooth and stress-free.
You Have the Right to Choose
Families may transfer GAPP services from one agency to another at any time. You are not locked into any provider and can select the agency that best fits your family's needs.
We Assist with the Transition
Our intake team will guide you through the transfer process, coordinate with your current provider, and help with any necessary documentation to ensure continuity of care.
No Interruption of Care
We work diligently to ensure your child's nursing care continues without gaps whenever possible, so your family experiences minimal disruption.
