What is GAPP?
The Georgia Pediatric Program (GAPP) is a Medicaid waiver program designed to support medically fragile children who require a hospital level of care but can safely receive that care at home with the right support.
GAPP enables children with complex medical needs — including those on ventilators, with feeding tubes, or requiring skilled nursing oversight — to remain in the comfort of their own homes and communities.
Prime Care Nursing Services is a GAPP-enrolled provider, bringing our clinical expertise and compassionate approach to families across DeKalb, Fulton, Clayton, Cobb, and Henry counties.

Ventilator Care

Medication Administration
Safe, accurate medication administration and management by licensed nurses, including injectable and enteral routes.
IV / PICC Line Management
Skilled nursing care for intravenous and PICC line maintenance, medication infusion, and site monitoring.
G-Tube & Enteral Feeding
Management of gastrostomy tubes and enteral feedings, including nutrition delivery and site care.
Skilled Nursing Assessments
Comprehensive RN and LPN assessments, clinical oversight, and health monitoring for medically fragile children.
Personal Care & ADL Support
Compassionate assistance with daily living activities tailored to each child's care plan.
Personal Care Assistance
Compassionate support with bathing, grooming, dressing, and personal hygiene tailored to each child's needs.
ADL Assistance
Hands-on assistance with activities of daily living including mobility, transfers, positioning, and feeding.
Respite Care
Temporary relief for family caregivers while ensuring continuity of safe, high-quality care for their child.
